Fenwal websites contain information about products and services which may or may not be available in any particular country, and which, if applicable, may have received approval or market clearance by a governmental regulatory body for different indications and restrictions in different countries. Each country has specific laws, regulations and medical practices governing the communication of medical or other information about medical products on the Internet. Nothing herein should be construed as a solicitation or promotion for any product or for an indication for any product that is not authorized by the laws and regulations of the country where the reader resides. Nothing on this site is intended to solicit sales in any jurisdiction where Fenwal is not already doing business.
This site may contain information on medical and health-related topics. Product information on this web site is provided by Fenwal and is intended for general information purposes only. Many pharmaceutical and medical device products listed are available upon prescription from a medical doctor or qualified medical professional only, and not all such products may be available in all countries. The product information is not intended to provide complete medical information. SHOULD YOU HAVE A MEDICAL CONDITION, PROMPTLY SEE YOUR OWN MEDICAL DOCTOR OR HEALTH CARE PROVIDER. FENWAL DOES NOT OFFER PERSONALIZED MEDICAL DIAGNOSIS OR PATIENT-SPECIFIC TREATMENT ADVICE. Information on these products may vary by country. Patients, physicians and other medical professionals should check with local medical resources and regulatory authorities for information appropriate to their country. In addition, current regulations in many countries limit (or even prohibit in some instances) the ability of Fenwal to provide information and/or to respond directly to patient questions regarding its prescription products. Fenwal, however, will respond to inquiries from and provide information to your qualified health care professional in accordance with local regulations.
This site provides a wide range of information to visitors, including information about products, product usage, product documentation, training tips, industry information, and our related services. Like many websites, this site also collects information from visitors, including personally identifiable information (your full name, e-mail address, phone number, and so on) and non-personally identifiable information (e.g., aggregate statistics about what parts of the site are most popular). This Policy is designed to tell you about information collection practices at this site, i.e., the ways we collect information; what kinds of information we collect; why we collect information; how we use information we collect; with whom we share information we collect; how you can access, amend, and delete information we collect from you; and what kinds of security we use to protect information you provide.
Scope of Policy
Active Information Collection
This site collects information from its visitors in two ways: actively and passively. Active information collection refers to instances in which we gather information from you when you fill in a form or request and send it to us, such as by completing a registration form, signing up for an electronic newsletter, or sending us a message via an e-mail reply mechanism provided by this site. Active information collection can include both personally identifiable information (e.g., your full name or a identification number unique to you) and non-personally identifiable information (e.g., if we were to ask for your first name and a list of the top three things you like about our site). Passive information collection refers to instances in which we collect information from you that you have not actively provided (please see the next section of this Policy for a summary of our passive information collection practices).
Below we have summarized the primary means of active information collection at our site. By submitting such information at these various information collection points, you specifically consent to the collection, use, and dissemination of such information in accordance with this Policy. At each of this site's active information collection points, we have marked which information is required and which is optional. Unless otherwise specified below, failure to provide required information might mean you will not be allowed to view restricted information but may proceed into the site to view public information. Failure to provide optional information will not prevent your participation in the identified activity or prevent your access to the site, but it may inhibit our ability to customize the site for you and better understand your interests and needs.
This site is intended to be used by non-registered visitors and registered visitors and customers located in the United States. Specific functions of this site and information will be unavailable to you without registration. If you are an active Fenwal customer and wish to register, please complete the registration form on the site. We will review your request, verify you are a Fenwal customer, and issue you an ID and password. We may also ask for additional information, such as your telephone, fax number, location or job title as part of the process to set-up customer users and to help us serve you better in the future. As part of the set-up process, we will also use customer information we already have on record for verification purposes and to enable you to use the site to access products and services. You acknowledge that you are responsible for all use of this site conducted via your user ID or password, including unauthorized use. If at any time you know or suspect that your personal user name or password has been compromised, you must immediately notify us at 1-800-3-FENWAL. We use the information provided to us through the registration and user set-up processes and to manage our site's operations and security.
Applying for an Open Position
The site allows you to submit a resume and cover letter to apply for open positions. Site registration is not required to be considered for employment. We ask that you provide personally identifiable information such as your name, e-mail address, title, resume, and phone number when applying for a position so that we may contact you to discuss potential employment. Fenwal will not contact current or past employers without your permission. Resumes are maintained for 90 days, during which time you may be considered for any open position for which you are qualified. You may request that your records be deleted by calling 1-800-3-FENWAL and indicate you applied for a position using the Careers application tool and would like for Fenwal to delete your records.
We welcome your contact regarding this site and our products, services, and programs. This site provides an e-mail reply mechanism that permits you to send us your e-mail address and any comments you choose to include.
This site also includes a feedback form for you to use in submitting comments; we do not ask you to provide personally identifiable information such as your name, e-mail address, title, and phone number when providing this feedback. The feedback form does ask you to respond to questions regarding your Internet usage habits or our site navigation. We use the information you decide to send us to follow up with you regarding your comments, improve our products and services, better understand our customers' needs, maintain and operate our site and guide future enhancements to this site and our Internet presence generally. Information you provide in a feedback form is generally optional, although you will need to provide us with contact information if you would like us to send a response.
Passive Information Collection
We may use special technologies to passively collect information while you are visiting our site (that is, you do not actively provide this information). These technologies allow us to collect certain kinds of non-personally identifiable information, such as your Internet domain name, the type of web browser you use, the operating system you use, the date and time you visit the site, the specific pages you visit on the site, and address of the website you visited prior to entering this site. We do not link this passively collected non-personally identifiable information with personally identifiable information you have actively provided elsewhere on this site. We reserve the right to share with third parties, including our business partners, the anonymous aggregated information that we have passively collected via this site.
Below we have summarized the primary means of passive information collection at our site.
This site uses "cookies," which are small text files that store non-personally identifiable information about you, either temporarily in connection with your Internet Protocol (IP) address (known as a temporary or session cookie, and deleted once you close your browser window) or more permanently on the hard drive of your computer (known as a permanent or persistent cookie). This site uses session cookies to store user IDs and identify you as a registered user. This site does not store passwords or other information. If you have set your browser to not accept cookies, you will still be able to sign-on but your browser will not automatically enter your user name, and you will need to enter it and your password every time to sign-onto this site. You may set your browser to reject cookies and let you know when a cookie is being placed on your computer.
Internet Protocol (IP) Addresses
This site uses Internet protocol (IP) addresses. IP addresses are unique identifiers automatically assigned to each computer when logging onto the Internet. IP addresses allow us to collect much of the information we collect through cookies, including browser-type, what pages you visit, and how long you stay on the site. In general, IP addresses allow us to identify problems with our servers, prepare aggregate demographic information and other information regarding the use of this site, and improve the functions of this site.
Other Passive Tracking at This Site
This site uses tracking and monitoring software to manage our servers and their availability. In addition, we use server logs and user logs to provide an audit trail of transactions that are executed on this site. This information helps us to operate and maintain the site as well as monitor security, possible misuse and unauthorized access or alteration. This site may also collect clickstream data and navigational data that will be compiled into statistical information that will be used in aggregate form to help us improve the site through system changes, enhancements or to help us better customize the site for your use. This information may be shared with qualified third parties but only for the stated purposes mentioned.
What We Do With Your Information
The personally identifiable information we collect from you will only be used and disclosed in accordance with this Policy or as specifically disclosed to you at the time you provide the information. We will not sell, lease, transfer, share, or otherwise permit access to your personally identifiable information with any third parties other than as described in this Policy or otherwise disclosed to you. We reserve the right to sell, lease, transfer, or otherwise share without restriction aggregated, de-identified, and other non-personally identifiable information, including site usage, sales, traffic patterns, demographics, and other statistics about our site or those from whom we collect information via this site. We reserve the right to use or disclose personal information provided via this site as necessary to respond to your requests, to comply with applicable laws, regulations, legal process, and law enforcement inquiries, to take precautions against liability, and to protect the security and integrity of the site and the safety of its users.
Fenwal may transfer any personally identifiable information to one of its databases outside the country. We will do our best to protect personal information that is transferred even if the country does not require Fenwal to provide adequate protection for your data.
We take reasonable steps to ensure that our record of the personally identifiable information you send us via this site is accurate, current, and complete. Toward that end, we provide you with reasonable access to your personal information to correct errors or delete the information you have provided. If you wish to correct or delete your personally identifiable information, please use the Site's feedback form or contact Fenwal at 1-800-3-FENWAL, or by mail at Fenwal, Three Corporate Drive, Lake Zurich, IL 60047, Attention: Fenwalinc.com. To respond to your request or inquiry, we may require follow-up or contact information to allow us to verify that you are the person authorized to view the information you are requesting. We may, at our discretion, refuse to allow you access to certain information if, in our view, to provide such access would jeopardize another person's privacy; would violate any applicable federal, state, or foreign law, regulation, or directive; or would unreasonably burden this Site.
We take reasonable steps to protect the personally identifiable information that you provide via this site; however, at present no site, including this site, can be considered 100% secure or error-free. Therefore, we do not, and cannot, guarantee the security or accuracy of the information we gather. One way we protect your personally identifiable information is through user IDs and passwords. This safety measure is only effective if you protect the secrecy of both your user ID and password. If you ever suspect your user ID or password may have been compromised or subject to unauthorized use, immediately contact Fenwal at 1-800-3-FENWAL. This site will use role-based security, limited access, firewalls, and SSL encryption to protect transmission of data.
This Site is Not Intended for Use by Children
This site is not directed to, and does not seek to collect personally identifiable information from, children under the age of 18.
If you have any questions, comments, concerns or complaints about this Policy, or the information practices of this Site, use the reporting process outlined in our Code of Conduct. Fenwal is committed to ensuring that complaints and disputes concerning Fenwal's privacy practices are investigated and resolved.
Changes to This Policy
We reserve the right to change this Policy at any time and in our sole discretion. If we do change the Policy, the new Policy will be posted here, along with its revision date, and all information collected following the posting of such changes will be subject to the new Policy. In the event that you do not consent to the terms and conditions of the new Policy, your user ID and password and associated site functionality will be revoked. We encourage you to check this Policy periodically and, at a minimum, before each instance in which you provide personally identifiable information via this site.
This Policy was last revised on November 6, 2008.
